Progress Reporting System

As you work towards your ultimate goals it is important that together we monitor your progress.

We use a monitoring system on which we enter your feedback after each session. 

An explanation of each of the seven categories is detailed in the bottom section.

Please think of each entry as a ‘snapshot in time’ based on the progress made since the previous therapy session.


Please enter a number between 1 & 10 that indicates where you feel you are.

For example, if you don’t feel you’re achieving much - place a line closer to the lower end (1)
          whereas if the achievement is fantastic then perhaps it will be nearer to the higher end (10).

Category explanations

The first three categories relate to the ‘three P’s for well-being & serotonin production


1) Thoughts - to be able to reflect in a positive way and recall good happenings, to be able to forecast positively

2) Interaction - to be able to interact in areas of choice in a positive and constructive way with family, friends, colleagues etc

Interaction is poor - 1                                                                                          10 - Interaction is good

3) Activity - to be able to create activities of choice or renew involvement in activities that were beneficial and enjoyable in the past.

I do nothing at present - 1                                                                                  10 - I am fully engaged

4) Confidence - to have that belief in oneself to achieve what one wants to achieve. Confidence is often thought of as the catalyst for change

Low confidence - 1                                                                                               10 - High Confidence

5) Strengths - an awareness of “what’s good about me” with new strengths and resources and renewing and re-establishing old ones. When we’re depressed we don’t recognise our strengths.

6) Achievement - what has been achieved in moving towards your “preferred future”.  Focus on what you’ve done well

7) Happiness – Overall Happiness
